Common Blood Tests Used in Menopause Care

Blood work can help clinicians evaluate menopausal symptoms, abnormal bleeding, and other health concerns that may overlap with menopause. While some tests assess hormone levels, others help identify medical conditions that can mimic or worsen menopausal symptoms.

Estradiol (E2), Follicle-Stimulating Hormone (FSH), and Luteinizing Hormone (LH)

These hormones provide information about ovarian function and reproductive health.

Why they're ordered:

  • Evaluate menstrual irregularities, infertility, and ovulation.
  • Help distinguish conditions such as:
    • Primary ovarian insufficiency (POI)
    • Hypothalamic amenorrhea
    • Polycystic ovary syndrome (PCOS)
  • Assess ovarian reserve and fertility concerns.

Important considerations:

  • Hormone levels fluctuate significantly during the menstrual cycle and throughout perimenopause.
  • Because of these fluctuations, FSH, LH, and estradiol are not reliable tests for determining where someone is in the menopausal transition.
  • Menopause is diagnosed clinically after 12 consecutive months without a menstrual period, not by a single hormone test.
  • Hormone testing may be useful for women who:
    • Have had a hysterectomy but still have their ovaries
    • Use a hormonal IUD and no longer have regular bleeding
  • Hormone levels may occasionally be checked to assess estrogen absorption during hormone therapy, but treatment decisions are based primarily on symptoms rather than laboratory values.
  • Results may be inaccurate in people using hormonal contraception that suppresses ovulation, including birth control pills, Depo-Provera, and Nexplanon.

Total and Free Testosterone

Testosterone testing can be helpful when evaluating symptoms related to androgen excess or low sexual desire.

Why it's ordered:

  • Assess for conditions such as PCOS and other causes of elevated androgen levels.
  • Evaluate symptoms including excess facial/body hair, acne, or hair thinning.
  • Support evaluation of low libido in select patients.

Key points:

  • Free testosterone is often more sensitive than total testosterone for detecting androgen excess.
  • Testosterone levels decline with age, but not solely because of menopause.
  • Women who retain their ovaries continue producing testosterone after menopause.
  • Menopause alone is not an indication for testosterone therapy.
  • Currently, the primary evidence-based use of testosterone therapy in postmenopausal women is treatment of hypoactive sexual desire disorder (low libido).

Thyroid Testing (TSH, Free T4, +/- T3)

Thyroid disorders can cause symptoms that closely resemble menopause.

Why it's ordered:

  • Evaluate fatigue, weight changes, mood changes, brain fog, palpitations, and sleep disturbances.

Why it matters:

  • Hypothyroidism may contribute to fatigue, depression, memory concerns, high cholesterol, and cardiovascular risk.
  • Hyperthyroidism may increase the risk of atrial fibrillation, bone loss, fractures, and heart disease.

Vitamin B12

Vitamin B12 is essential for healthy blood cells and nervous system function.

Why it's ordered:

  • Evaluate fatigue, anemia, peripheral neuropathy, memory concerns, or mood changes.
  • Screen patients at higher risk for deficiency, including:
    • Vegans and vegetarians
    • Individuals with gastrointestinal disorders
    • Those taking metformin, proton-pump inhibitors (e.g., omeprazole), or H2 blockers (e.g., famotidine)

Key points:

  • Low levels can contribute to fatigue, numbness, tingling, anemia, and cognitive symptoms.
  • Borderline results may require additional testing.
  • Persistently elevated B12 levels—particularly in people not taking supplements—may warrant further medical evaluation.

Vitamin D

Vitamin D plays an important role in bone, muscle, and immune health.

Why it's ordered:

  • Assess bone health and osteoporosis risk.
  • Evaluate individuals at risk for deficiency.

Why it matters:

  • Low vitamin D levels are associated with:
    • Reduced bone density
    • Increased fracture risk
    • Higher rates of metabolic and cardiovascular disorders
  • Excessively high vitamin D levels are uncommon but can lead to elevated calcium levels and kidney stones.

Ferritin (Iron Stores)

Ferritin reflects the body's iron storage levels.

Why it's ordered:

  • Evaluate fatigue, hair loss, restless legs syndrome, dizziness, or anemia.
  • Assess iron deficiency or iron overload.

Key points:

  • Low ferritin may indicate iron deficiency, often due to inadequate intake or blood loss.
  • Iron deficiency can cause fatigue, impaired concentration, hair loss, headaches, and restless legs syndrome.
  • High ferritin may reflect inflammation or, less commonly, iron overload conditions such as hereditary hemochromatosis.
  • Ferritin levels generally increase after menopause and may be associated with metabolic health changes.

Zinc

Zinc supports immune health, brain function, and mood regulation.

Why it's ordered:

  • Evaluate possible deficiency in individuals with mood symptoms, poor dietary intake, malabsorption, alcohol use, or medication-related risk factors.

Key points:

  • Low zinc levels have been associated with depressive symptoms.
  • Some studies suggest zinc supplementation may improve mood, particularly when used alongside standard depression treatments.

Omega-3 Index

The omega-3 index measures the amount of EPA and DHA in red blood cell membranes and reflects long-term omega-3 status.

Why it's ordered:

  • Assess cardiovascular health and guide dietary or supplement recommendations.

Key points:

  • Lower omega-3 levels are associated with higher cardiovascular risk.
  • Higher levels are linked to improved heart health outcomes.
  • Some evidence suggests omega-3 supplementation may help reduce menopausal night sweats.

Takeaway

While hormone testing has a limited role in diagnosing menopause, blood work can be valuable for identifying underlying conditions that may contribute to symptoms such as fatigue, brain fog, mood changes, abnormal bleeding, low libido, hair loss, and sleep disturbances. Treatment decisions should focus on symptoms, medical history, and overall health—not laboratory values alone.
